The Petitioner herein has filed with the Clerk of the above-entitled Court a Petition for Termination of Parent-Child Relationship as to Father and for Adoption requesting the following relief all under Chapter 26.33 RCW:
A. An involuntary termination of the parent-child relationship between you, as father, and the minor child CARTER LANE WASISCO; and
B. Adoption of the minor child by the Petitioner who is the spouse of the mother.
This Summons and Notice of Hearing concerns CARTER LANE WASISCO, born July 29, 2006, in Bellingham, Washington. You are identified on the birth certificate as the father.
1. Summons. You may respond to this Summons and the Petition by filing a written response with the Clerk of the Court and serving a copy of your response on the person signing this Summons. If you do not serve a written response within twenty (20) days after the date this Summons was served on you or first published, exclusive of the day of service or first publication, the Court may enter an Order of Default against you. After default, the Court may, without further notice to you, enter an Order terminating your parent-child relationship with and all your rights with respect to the child, and approve an adoption of the minor child.

2. Notice of Hearing. A court hearing will be held on the termination petition on the 29th day of June, 2015, at 9:00 a.m. in Department 3 of the Whatcom County Superior Court, Whatcom County Courthouse, 311 Grand Ave., Bellingham, WA 98225. Your failure to appear at this hearing may result in the issuance of an Order of Default. After default, the Court may, without further notice to you, enter an Order terminating your parent-child relationship with and all your rights with respect to the child, and approve an adoption of the minor child.
